Register for our Comprehensive Membership
Our membership is designed to allow for continuity of care, and it is the only way to be seen by our clinicians. Your membership will start on the date of your first visit. Please be sure to book your appointment after you’ve registered for your membership. After you register, you will be invited to our secure messaging platform. Whitney is accepting new patients in Cornelius Monday-Wednesday and Winston Salem on Thursdays. Christina has limited number of new patient slots, and sees new patients on Wednesdays.

Next, Submit Your Medical History Paperwork
After you have registered for a membership , we kindly request you submit your new patient paperwork promptly. We do review your paperwork well in advance of your first appointment, so that if there is additional information we need from you we can reach out.
