STEP 1
Register for the Membership, or Pay for a Single Consultation Appointment
How to Decide: For most of our new patients seeking regular routine care, the membership makes the most sense financially because you have access to as many appointments as you need for your situation. If you are still uncertain, we recommend booking an initial paid consultation appointment and you can decide to join the membership for your follow ups (registration fee waived).
STEP 2
Next, Submit Your Medical History Paperwork
After you have registered for a membership, or booked a paid consultation appointment you need to create an account on IntakeQ and submit your medical paperwork and history to us. We do review your paperwork before your first appointment, so please submit it in advance of your first appointment.
STEP 3
Lastly, Choose Your Appointment Time
After steps 1 and 2 are completed, we will have you create an account through Athena Health and schedule your appointment time.
