Continuous glucose monitoring (CGM) can be extraordinarily helpful in individuals with blood sugar issues such as hypoglycemia, prediabetes, and diabetes. Your CCWW clinician may either prescribe you a CGM that you’d pick up at your retail pharmacy, or recommend you purchase a direct-to-consumer device such as Stelo. The product you get will have information about downloading an app and how to get set up using your device.

If you are a current patient and are using either Dexcom or Stelo, you can connect to our clinics dashboard through your app using the clinic code CCWW2022.
